****Pet ownership is a big responsibility. The policy of Friends of Animals Rice County, Inc. (FOARC) is to insure that each person who adopts a pet is aware of this responsibility and that they are morally, financially, and physically capable of accepting it.

Please read the following statements and initial under each if you agree.

I will provide proper and adequate food, water, housing, exercise, and grooming for this pet and will treat him/her humanely at all times. *

(A) I will provide veterinary care in the form of annual vaccinations, preventative heartworm medications and flea and tick preventative, as well as veterinary care that is necessary to prevent and/or treat accidents and illnesses. *

(A) If it is determined that this pet has not been spayed or neutered, I will have him/her spayed or neutered before he/she reaches six months of age. *

I have been informed that all animals can carry and transmit diseases, some of which can affect people, including bacteria, viruses, parasites, and ringworm and that these diseases may be undetectable in what appears to be a healthy animal at the time of adoption. *

FOARC is in no way responsible for any damage that the animal may inflict on another person, animal, or property, and I will make no attempt to hold FOARC responsible. *

(A) I accept the animal as it is at the time of adoption and understand that FOARC is not responsible for any medical conditions not readily detected prior to or at the time of this adoption or discovered after such adoption. *

I will obey local licensing and animal confinement laws. *

I will not sell, give away, or use this animal for experimental purposes, allow him/her to engage in dog fighting, or pursue any guard dog or attack dog training with him/her. *

My place of residence allows for the ownership and occupancy of the pet I am adopting. *

I am aware that pets may exhibit normal, but potentially undesirable behaviors. These behaviors include but are not limited to: aggression, house soiling, biting, scratching (people, furniture, and woodwork), barking, digging, mounting people’s legs, marking with urine or spraying urine and that these normal behavior patterns may be difficult to manage. No one at this facility has told me that the pet will not engage in any of these behavior patterns. *

(A) I am aware that it usually costs between $250 and $750 per year to feed, house, train, and provide veterinary care for a pet, and I am financially able to meet these expenses for my adoptive pet. *

I have read this agreement and release FOARC from any present or future liability associated with my adoption of this animal. *

By signing this application, you agree that all of the above information is accurate. You agree to an adoption fee of $200 for dogs or $150 for cats (in special circumstances, this fee may be changed by FOARC to accommodate any extra expenses of adopted pet). The adoption fee includes the spay or neuter of pet (if old enough at time of adoption), age appropriate vaccinations, microchip, and heartworm/feline leukemia test prior to adoption.
